How to build AI agents that don’t break at scale

The early success of AI tools is creating an illusion of readiness and scale that many organizations are not yet equipped to roll out or sustain.

What’s possible in a couple of carefully selected pilots is rarely applicable to large-scale deployments.

As you scale, workflows become less predictable, attention is spread thin and issues are not caught as quickly as before, which makes things more fragile.

Let’s look at where these gaps appear most often, and what needs to change for AI agents to hold up as expected once they are part of everyday work.

Unclear goals make agents less effective

While AI agent pilots are forgiving and people are more hands-on, one of the biggest issues with any scaling a project is starting without a clear goal. When teams don’t define exactly what they want an AI agent to do, the result is often something that feels unfocused or doesn’t solve a real business problem.

In fact, Gartner claims that over 40% of agentic AI projects will be cancelled by the end of 2027, due to escalating costs, unclear business value or inadequate risk controls.

The teams that see the best results start small and specific. They choose one clear task to automate and set simple expectations, which makes an AI agent easier to train and improve over time. This approach accelerates early wins and provides a clear blueprint for scaling an AI agent into other areas of the business.

Weak data foundations hurt performance

AI agents depend on accurate and up-to-date data. If the data feeding the system is messy or inconsistent, even a great model will struggle. In fact, Gartner predicts that through 2026, organizations will abandon 60% of AI projects unsupported by AI-ready data.

First, leaders must define what constitutes AI-ready data. Then they must ensure that the data is representative of the AI use case, know whether it is interoperable across the business, how the data should be protected when being fed into AI models, and have a system for automatically detecting sensitive data.

Then data teams must prepare data pipelines to build an AI model dataset for training and for the live data feed to AI production systems based on the requirements gathered and once ready test and monitor it to optimize the models.

Thereafter, data observability processes are then needed to track data patterns and changes, adjusting data requirements as needed.

Lack of transparency erodes trust

Organizations must choose tools that provide visibility into an AI agent’s reasoning and behavior. As soon as AI agent projects are out of the pilot phase, it's not possible for humans to oversee everything. Transparency has to be embedded as an operating feature so that things can be debugged, updated, relied upon and trusted.

Executives are increasingly recognizing the value of AI observability. Platforms and frameworks that surface an AI agent’s reasoning, highlight anomalies, avoid context decay, and give business leaders confidence that the system is behaving as intended.

Stress-testing transparency as you would performance is a must. Instead of asking, “Does this make sense to the team that built it?”, the question should be, “Would this make sense to someone encountering it for the first time six months from now?”

Poor integration slows everything down

AI agents don’t work well in isolation. Even the most capable AI agent cannot deliver value if it cannot interact and orchestrate with the systems that drive the business. They need to talk and take action among systems a company already relies on — CRMs, ERPs, workflow tools, data platforms, and even older on-premise software.

Leaders should view integration as a strategic, composable design structure rather than a post-deployment task.

They must prioritize platforms that can connect seamlessly across modern cloud systems, traditional enterprise applications and legacy infrastructure. The result is not just an AI agent that works — but one that feels native to the organization's existing workflow ecosystem.

Security and governance come too late

As AI agents take on more important tasks, they often handle sensitive business or customer data. Still, many teams only start thinking about security after the AI agent is built.

The strongest approach is to embed security and governance early, such as access controls, audit trails, data protections, and live monitoring. This keeps AI agents safe and predictable as they grow so that what they reason, plan, and act upon is known.

Be explicit about what the agent is allowed to do on its own and where it must always pause and bring a person in. And don’t lock those choices in on day one and forget them. Watch where teams naturally step in or override an AI agent, because that’s usually telling you something important.

It is a company's responsibility to know how its agents are behaving just like with its employees. This proactive stance not only mitigates risk but also accelerates adoption by giving stakeholders confidence that the system is secure and governed to use at scale.

AI agents can’t adapt when business needs change

Business priorities, mandates, rules and policies shift all the time, and AI agents need to keep up. If they can’t evolve, they quickly become outdated. Without intentional mechanisms for retraining, evaluation and feedback, an AI agent that was once well-aligned can quickly become outdated.

AI agents must be treated as living systems that are continuously reviewed. Teams should gather feedback, update models and regularly review performance so that an AI agent keeps improving and is aligned with the current business so they remain strategic assets.

Building AI agents that last

As AI moves deeper into core operations, the organizations that succeed won’t simply deploy AI agents — they’ll cultivate them. Success depends on how honest you are with every AI Agent initiative from the very beginning.

Always ask whether you have done enough in the set up stages, if there are any gaps and if you are truly ready to scale. Accept any pitfalls upfront and act on them, and bring in third party partners as necessary to help at every stage.
